Can cause dehydration and hypotension, especially in elderly, on diuretics, or with low BP; maintain fluids. Monitor kidney function; avoid/initiate with caution in renal impairment as per clinician advice. Higher risk of genital mycotic infections and UTIs; maintain hygiene and report symptoms. Temporarily withhold before major surgery/acute serious illness or prolonged fasting to reduce DKA risk. Use caution in patients with history of ketoacidosis.
