Composition of Acitrom 2 Tablet

Acitrom 2 Tablet contains Acenocoumarol (2mg). It belongs to the Vitamin K antagonist (coumarin derivative) class of medicines.

Uses of Acitrom 2 Tablet

• Prevention and treatment of blood clots (thromboembolism) such as deep vein thrombosis (DVT)
• pulmonary embolism (PE); prevention of stroke/systemic embolism in atrial fibrillation; prevention of clotting in patients with mechanical heart valves; secondary prevention after myocardial infarction where anticoagulation is indicated

How Acitrom 2 Tablet works

Acenocoumarol is a vitamin K antagonist anticoagulant. It inhibits vitamin K–dependent synthesis of clotting factors II, VII, IX and X in the liver, thereby reducing the blood’s ability to form clots. Its effect is monitored using INR and dose is adjusted accordingly.

How to take Acitrom 2 Tablet

Take exactly as prescribed by your doctor; dose is individualized based on INR. Usually taken once daily at the same time each day, with or without food. Do not stop, skip, or double doses; if a dose is missed, take it the same day as soon as remembered (do not double the next day). Regular INR testing is essential.

Take it exactly as your doctor prescribed. Do not change the dose or stop early on your own.

Missed dose and overdose

If you miss a dose

Take it as soon as you remember. If your next dose is almost due, skip the one you missed — never take two doses together to catch up.

If you take too much

An overdose of this medicine can cause serious and difficult-to-control bleeding. Do not wait for symptoms to appear — go to the nearest emergency department and take the pack or strip with you.

Side effects of Acitrom 2 Tablet

Most side effects of Acitrom are mild and settle as the body adjusts. Reported side effects include:

• Bleeding (nosebleeds

• prolonged bleeding from cuts
• blood in urine
• black/tarry stools
• nausea
• vomiting
• diarrhea
• abdominal pain
• rash
• hair loss
• dizziness

Stop the medicine and seek help immediately for a rash, swelling of the face or lips, wheezing or difficulty breathing. Tell your doctor about any side effect that persists or worries you.

Warnings and precautions

Risk of serious/life-threatening bleeding—seek urgent care for severe headache, weakness, coughing/vomiting blood, black stools, heavy menstrual bleeding, or any unusual bleeding. Requires regular INR monitoring and dose adjustments. Inform doctor before any surgery/dental procedure; may need temporary stoppage/bridging. Maintain consistent dietary vitamin K intake (avoid sudden changes in green leafy vegetables); avoid OTC NSAIDs/aspirin unless advised. Use caution in liver/kidney disease, uncontrolled hypertension, history of bleeding, peptic ulcer, alcoholism, elderly. Contraindicated in active bleeding, severe bleeding disorders, recent major surgery with high bleeding risk, and known hypersensitivity.

Do not take Acitrom if

Not for anyone with a known allergy to this medicine or to any ingredient in the pack. Not for people with active bleeding, a bleeding disorder, or a recent bleed into the brain.

Interactions with other medicines

Aspirin and other antiplatelets (clopidogrel), NSAIDs (ibuprofen, diclofenac), other anticoagulants (heparin, enoxaparin, dabigatran/rivaroxaban/apixaban), antibiotics (metronidazole, co-trimoxazole, macrolides, fluoroquinolones), antifungals (fluconazole, ketoconazole), amiodarone, statins (some), anticonvulsants (phenytoin, carbamazepine), rifampicin, thyroid hormones, corticosteroids, SSRIs/SNRIs, herbal products (ginkgo, garlic, ginseng, St John’s wort), vitamin K supplements

How to store Acitrom 2 Tablet

Store below 25°C in a dry place, protected from light and moisture. Keep out of reach of children.

Expert advice for Acitrom 2 Tablet

Carry a card or note saying which blood thinner you take. Take it at the same time every day; a missed dose leaves you less protected. Use paracetamol rather than an anti-inflammatory for ordinary aches, unless your doctor says otherwise.
