Telmisartan is an angiotensin II receptor blocker (ARB) that relaxes blood vessels by blocking angiotensin II–mediated vasoconstriction and aldosterone release. Hydrochlorothiazide is a thiazide diuretic that increases salt and water excretion, lowering blood volume and blood pressure.
Take orally as prescribed, usually once daily at the same time each day; can be taken with or without food. Swallow tablet with water; do not stop abruptly without medical advice. If a dose is missed, take it when remembered unless it is close to the next dose.
Common side effects of TELLME H TAB may include:
Not recommended in pregnancy (especially 2nd/3rd trimester); may cause dizziness—use caution while driving. Monitor kidney function and electrolytes (potassium, sodium) especially in elderly, dehydration, vomiting/diarrhoea, or renal impairment. Use with caution in liver disease, gout, diabetes, and in patients with low blood pressure/volume depletion. Risk of angioedema (rare); seek urgent care for facial/lip/tongue swelling. Can worsen kidney function in bilateral renal artery stenosis. Photosensitivity may occur with hydrochlorothiazide.
