Composition of Tellme H Tablet

Tellme H Tablet contains Telmisartan 40 mg + Hydrochlorothiazide 12.5 mg. It belongs to the Angiotensin II receptor blocker (ARB) + Thiazide diuretic class of medicines.

How Tellme H Tablet works

Telmisartan is an angiotensin II receptor blocker (ARB) that relaxes blood vessels by blocking angiotensin II–mediated vasoconstriction and aldosterone release. Hydrochlorothiazide is a thiazide diuretic that increases salt and water excretion, lowering blood volume and blood pressure.

How to take Tellme H Tablet

Take orally as prescribed, usually once daily at the same time each day; can be taken with or without food. Swallow tablet with water; do not stop abruptly without medical advice. If a dose is missed, take it when remembered unless it is close to the next dose.

Take it exactly as your doctor prescribed. Do not change the dose or stop early on your own.

Missed dose and overdose

If you miss a dose

Take it as soon as you remember. If your next dose is almost due, skip the one you missed — never take two doses together to catch up.

If you take too much

An overdose of this medicine can cause a severe fall in blood pressure, with dizziness, fainting and a fast or slow pulse. It can cause severe dehydration, a sharp fall in blood pressure and dangerous salt imbalances. Do not wait for symptoms to appear — go to the nearest emergency department and take the pack or strip with you.

Side effects of Tellme H Tablet

Most side effects of Tellme H are mild and settle as the body adjusts. Reported side effects include:

• Dizziness
• headache
• fatigue
• increased urination
• nausea
• low blood pressure
• electrolyte imbalance (low potassium/low sodium)
• increased uric acid
• increased blood glucose
• dehydration

Stop the medicine and seek help immediately for a rash, swelling of the face or lips, wheezing or difficulty breathing. Tell your doctor about any side effect that persists or worries you.

Warnings and precautions

Not recommended in pregnancy (especially 2nd/3rd trimester); may cause dizziness—use caution while driving. Monitor kidney function and electrolytes (potassium, sodium) especially in elderly, dehydration, vomiting/diarrhoea, or renal impairment. Use with caution in liver disease, gout, diabetes, and in patients with low blood pressure/volume depletion. Risk of angioedema (rare); seek urgent care for facial/lip/tongue swelling. Can worsen kidney function in bilateral renal artery stenosis. Photosensitivity may occur with hydrochlorothiazide.

Do not take Tellme H if

Not for anyone with a known allergy to this medicine or to any ingredient in the pack. Must not be used in pregnancy — it can seriously harm the developing baby. Not for people with narrowing of both kidney arteries, or a history of swelling of the face and throat caused by this group of medicines. Not for people who are unable to pass urine, or who are severely dehydrated.

Interactions with other medicines

ACE inhibitors or aliskiren (in diabetes/renal impairment) increased risk of kidney problems and high potassium, potassium supplements/potassium-sparing diuretics or salt substitutes (hyperkalaemia), NSAIDs (reduced antihypertensive effect and kidney risk), lithium (increased lithium toxicity), other antihypertensives (additive low BP), corticosteroids/amphotericin B (electrolyte loss), antidiabetic medicines (may require dose adjustment), digoxin (electrolyte changes may increase arrhythmia risk)

Store below 25°C, protected from moisture and light; keep in original strip; keep out of reach of children.

Expert advice for Tellme H Tablet

Blood pressure medicines work only while you take them — do not stop because you feel well. Tell your doctor at once if you become pregnant or are planning a pregnancy. Keep a home blood-pressure record if your doctor has asked for one. Weigh yourself at the same time each day if you have heart failure, and report a rapid gain. Do not add extra salt, and ask before using a potassium-based salt substitute.
