Metformin (1000mg) + Dapagliflozin (10mg) + Sitagliptin (100mg)
Type 2 diabetes mellitus (as an adjunct to diet and exercise) to improve glycaemic control when triple therapy is appropriate.
Metformin reduces glucose production by the liver and improves insulin sensitivity. Sitagliptin (a DPP-4 inhibitor) increases incretin levels, enhancing insulin release and reducing glucagon after meals. Dapagliflozin (an SGLT2 inhibitor) increases urinary glucose excretion by blocking glucose reabsorption in the kidneys.
Oral tablet; take exactly as prescribed. Usually taken once daily with food (preferably with the main meal) and swallowed whole (do not crush/chew) if extended-release. Maintain adequate hydration and follow diet/exercise advice; monitor blood glucose as advised.

Not for type 1 diabetes or diabetic ketoacidosis. Risk of lactic acidosis with metformin (higher in kidney impairment, dehydration, severe infection, alcohol excess); assess renal function before and during therapy. Dapagliflozin may cause volume depletion/low BP, genital/urinary infections, and rare ketoacidosis (may occur even with near-normal glucose); stop and seek care if nausea/vomiting, abdominal pain, rapid breathing occur. Sitagliptin has rare risk of pancreatitis and severe joint pain; seek care for severe abdominal pain. Use caution in elderly, patients on diuretics, low BP, or with renal/hepatic impairment; hold around major surgery/acute illness and before iodinated contrast as per clinician advice. Monitor for hypoglycaemia if used with insulin/sulfonylurea.
