Bilasure 40 M Tablet contains Telmisartan 40 mg + Metformin (prolonged-release) 500 mg. It belongs to the Angiotensin II receptor blocker (ARB) + Biguanide class of medicines.

How Bilasure 40 M Tablet works

Telmisartan is an angiotensin II receptor blocker (ARB) that relaxes blood vessels and lowers blood pressure. Metformin reduces hepatic glucose production and improves insulin sensitivity, helping lower blood glucose.

How to take Bilasure 40 M Tablet

Take as directed by physician, usually once daily with food (metformin component). Swallow whole; do not crush or break prolonged-release tablets. Maintain diet and exercise as advised.

Take it exactly as your doctor prescribed. Do not change the dose or stop early on your own.

Missed dose and overdose

If you miss a dose

Take it as soon as you remember. If your next dose is almost due, skip the one you missed — never take two doses together to catch up.

If you take too much

An overdose of this medicine can cause a severe fall in blood pressure, with dizziness, fainting and a fast or slow pulse. It can cause a dangerous build-up of acid in the blood, with vomiting, severe weakness and laboured breathing. Do not wait for symptoms to appear — go to the nearest emergency department and take the pack or strip with you.

Most side effects of Bilasure are mild and settle as the body adjusts. Reported side effects include:

Warnings and precautions

Monitor kidney function and potassium periodically. Metformin may rarely cause lactic acidosis (higher risk in severe kidney/liver disease, dehydration, heavy alcohol use, or hypoxia). Avoid in pregnancy unless specifically advised; caution in elderly and in patients with heart failure. Temporarily stop metformin before iodinated contrast procedures or major surgery as advised by doctor.

Do not take Bilasure if

Not for anyone with a known allergy to this medicine or to any ingredient in the pack. Must not be used in pregnancy — it can seriously harm the developing baby. Not for people with narrowing of both kidney arteries, or a history of swelling of the face and throat caused by this group of medicines. Not for people with severe kidney disease, severe liver disease, or an acute illness causing dehydration. Not for people who have had diabetic ketoacidosis.

Interactions with other medicines

ACE inhibitors/other ARBs, potassium supplements or potassium-sparing diuretics (e.g. spironolactone), NSAIDs (may affect kidneys/BP), diuretics, lithium, insulin or other antidiabetics (hypoglycemia risk), alcohol (lactic acidosis risk), iodinated contrast media
