Sitagliptin 50 mg + Metformin hydrochloride 500 mg (tablet)
Type 2 diabetes mellitus: to improve blood glucose control in adults when diet and exercise alone or single-drug therapy is inadequate
Sitagliptin is a DPP-4 inhibitor that increases incretin hormones (GLP-1, GIP), leading to increased glucose-dependent insulin release and reduced glucagon levels. Metformin reduces hepatic glucose production and improves peripheral insulin sensitivity, lowering blood glucose without usually causing hypoglycaemia when used alone.
Take by mouth exactly as prescribed. Usually taken with meals to reduce stomach upset (commonly twice daily). Swallow whole with water; do not crush/chew unless advised. Follow diet, exercise and regular blood sugar monitoring as recommended by your doctor.

Not for type 1 diabetes or diabetic ketoacidosis. Metformin may rarely cause lactic acidosis—risk increases with kidney impairment, dehydration, severe infection, liver disease, heavy alcohol use, or hypoxia (heart/respiratory failure); seek urgent care for rapid breathing, severe weakness, abdominal pain or drowsiness. Assess kidney function before and during therapy; dose adjustment/avoid in significant renal impairment. Sitagliptin has been associated rarely with pancreatitis—stop and seek care for severe persistent abdominal pain. Use caution in elderly, those with heart failure, and around surgery or iodinated contrast procedures (metformin may need temporary discontinuation).
