Composition of Telemar 40 H

Telemar 40 H contains Telmisartan (40mg) + Hydrochlorothiazide (12.5mg). It belongs to the Angiotensin II receptor blocker (ARB) + Thiazide diuretic class of medicines.

How Telemar 40 H works

Telmisartan is an angiotensin II receptor blocker (ARB) that relaxes blood vessels by blocking angiotensin II, lowering blood pressure. Hydrochlorothiazide is a thiazide diuretic that increases salt and water excretion through urine, reducing fluid volume and BP. Together they provide additive blood pressure control.

How to take Telemar 40 H

Oral tablet. Take once daily as prescribed, preferably at the same time each day, with or without food. Swallow whole with water; do not stop suddenly without medical advice. Diuretic component may increase urination—taking in the morning may help avoid night-time urination.

Take it exactly as your doctor prescribed. Do not change the dose or stop early on your own.

Missed dose and overdose

If you miss a dose

If you miss a dose of Telemar H 40mg Tablet, take it as soon as possible. However, if it is almost time for your next dose, skip the missed dose and go back to your regular schedule. Do not double the dose.

If you take too much

An overdose of this medicine can cause a severe fall in blood pressure, with dizziness, fainting and a fast or slow pulse. It can cause severe dehydration, a sharp fall in blood pressure and dangerous salt imbalances. Do not wait for symptoms to appear — go to the nearest emergency department and take the pack or strip with you.

Side effects of Telemar 40 H

Most side effects of Telemar are mild and settle as the body adjusts. Reported side effects include:

• Dizziness
• headache
• fatigue
• increased urination
• low blood pressure
• nausea
• muscle cramps
• increased blood uric acid
• electrolyte imbalance (low sodium/low potassium)
• increased creatinine

Stop the medicine and seek help immediately for a rash, swelling of the face or lips, wheezing or difficulty breathing. Tell your doctor about any side effect that persists or worries you.

Warnings and precautions

Not recommended in pregnancy; can harm the unborn baby—stop and consult doctor if pregnant. Use caution in kidney disease, renal artery stenosis, liver disease, dehydration/low salt state, gout, diabetes, or history of electrolyte imbalance. May cause dizziness—avoid driving until you know the effect. Periodic monitoring of blood pressure, kidney function (creatinine), and electrolytes (potassium/sodium) is advised. Seek urgent care for swelling of face/lips/tongue (angioedema), severe lightheadedness, or signs of dehydration.

Do not take Telemar if

Not for anyone with a known allergy to this medicine or to any ingredient in the pack. Must not be used in pregnancy — it can seriously harm the developing baby. Not for people with narrowing of both kidney arteries, or a history of swelling of the face and throat caused by this group of medicines. Not for people who are unable to pass urine, or who are severely dehydrated.

Interactions with other medicines

ACE inhibitors or aliskiren (increased risk of kidney impairment, high potassium, low BP—avoid especially in diabetes), potassium supplements/potassium-sparing diuretics (risk of high potassium), NSAIDs (may reduce BP effect and worsen kidney function), lithium (increased lithium toxicity), other antihypertensives (additive BP lowering), diuretics/corticosteroids (electrolyte disturbances), antidiabetic medicines (may alter glucose control), digoxin (electrolyte changes may increase arrhythmia risk)

How to store Telemar 40 H

Store at room temperature (below 25°C), protected from moisture and light. Keep in original blister pack and out of reach of children.

Expert advice for Telemar 40 H

It can be taken with or without food. Telemar H 40mg Tablet may cause dehydration. Drink plenty of fluids and inform your doctor if you develop extreme thirst, very dry mouth or muscle weakness. It may cause dizziness. Get up slowly when rising from a sitting or lying position. Use of Telemar H 40mg Tablet during pregnancy or breastfeeding is unsafe.
