Telmisartan 40 mg + Amlodipine 5 mg + Hydrochlorothiazide 12.5 mg (per tablet)
Treatment of hypertension (high blood pressure), especially when blood pressure is not adequately controlled with single or dual therapy; helps reduce risk of cardiovascular events associated with uncontrolled BP.
Telmisartan blocks angiotensin II (ARB), relaxing blood vessels. Amlodipine (calcium channel blocker) further dilates arteries. Hydrochlorothiazide is a thiazide diuretic that removes excess salt and water, lowering blood pressure.
Take orally as prescribed, usually once daily at the same time each day; can be taken with or without food. Swallow whole with water. Do not stop abruptly without medical advice; monitor BP regularly.

May cause dizziness—avoid driving until you know its effects. Monitor kidney function and electrolytes (sodium, potassium) periodically. Use with caution in kidney/liver disease, dehydration, low BP, heart failure, aortic stenosis, or history of angioedema. Thiazides can increase uric acid (may worsen gout) and affect blood sugar; monitor in diabetes. Avoid in pregnancy; stop immediately if pregnant. Seek urgent care for facial/lip swelling or severe breathing difficulty.
