Telmisartan 40 mg + Chlorthalidone 12.5 mg (per tablet)
Treatment of hypertension (high blood pressure), especially when single medicine is not enough; helps reduce risk of cardiovascular events like stroke and heart attack by controlling BP.
Telmisartan is an angiotensin II receptor blocker (ARB) that relaxes blood vessels by blocking angiotensin II, lowering blood pressure. Chlorthalidone is a thiazide-like diuretic that increases salt and water excretion via kidneys, reducing blood volume and BP.
Take by mouth as prescribed, usually once daily at the same time each day. Can be taken with or without food; take preferably in the morning to reduce nighttime urination. Do not stop abruptly without medical advice.

Not recommended in pregnancy (can harm fetus). Use caution in kidney or liver disease, dehydration, low sodium/potassium, gout, diabetes, and elderly. May cause dizziness—avoid driving if affected. Monitor blood pressure, kidney function and electrolytes (potassium/sodium), and blood glucose/uric acid. Seek help for severe dizziness, fainting, swelling of face/lips, or reduced urine output.
