Telmifree 40 mg Tablet contains Telmisartan 40 mg. It belongs to the Angiotensin II receptor blocker (ARB) class of medicines.

How Telmifree 40 mg Tablet works

Telmisartan is an angiotensin II receptor blocker (ARB). It blocks AT1 receptors, preventing angiotensin II–mediated vasoconstriction and aldosterone release, thereby lowering blood pressure and reducing cardiovascular strain.

How to take Telmifree 40 mg Tablet

Take by mouth as prescribed, usually once daily, with or without food. Take at the same time each day; do not stop abruptly without medical advice. Dose adjustments may be needed in kidney/liver problems or if used with diuretics.

Take it exactly as your doctor prescribed. Do not change the dose or stop early on your own.

Missed dose and overdose

If you miss a dose

Take it as soon as you remember. If your next dose is almost due, skip the one you missed — never take two doses together to catch up.

If you take too much

An overdose of this medicine can cause a severe fall in blood pressure, with dizziness, fainting and a fast or slow pulse. Do not wait for symptoms to appear — go to the nearest emergency department and take the pack or strip with you.

Most side effects of Telmifree are mild and settle as the body adjusts. Reported side effects include:

• low blood pressure
• increased potassium (hyperkalemia)
• nausea

Stop the medicine and seek help immediately for a rash, swelling of the face or lips, wheezing or difficulty breathing. Tell your doctor about any side effect that persists or worries you.

Warnings and precautions

Not recommended in pregnancy (especially 2nd/3rd trimester) due to risk of fetal harm; avoid in breastfeeding unless advised. Use caution in kidney disease, renal artery stenosis, dehydration/low salt (may cause marked BP fall), and liver impairment. Monitor blood pressure, kidney function (creatinine) and serum potassium, especially after starting or dose changes. Do not use with aliskiren in diabetes; avoid dual blockade with ACE inhibitors/other ARBs unless specifically advised.

Do not take Telmifree if

Not for anyone with a known allergy to this medicine or to any ingredient in the pack. Must not be used in pregnancy — it can seriously harm the developing baby. Not for people with narrowing of both kidney arteries, or a history of swelling of the face and throat caused by this group of medicines.

Interactions with other medicines

Potassium supplements/salt substitutes and potassium-sparing diuretics (spironolactone, eplerenone, amiloride) increase hyperkalemia risk; NSAIDs may reduce effect and worsen kidney function; lithium levels may increase; other antihypertensives/diuretics can increase hypotension; ACE inhibitors/aliskiren increase kidney and potassium risks.

Store below 25°C in a dry place, protected from moisture and direct light. Keep in original blister pack and out of reach of children.

Blood pressure medicines work only while you take them — do not stop because you feel well. Tell your doctor at once if you become pregnant or are planning a pregnancy. Keep a home blood-pressure record if your doctor has asked for one.
