Sitagliptin 100 mg + Pioglitazone 15 mg + Metformin 500 mg (as Metformin Hydrochloride)
Type 2 diabetes mellitus—adjunct to diet and exercise to improve glycaemic control in adults when single/dual therapy is inadequate.
Metformin decreases hepatic glucose production and improves peripheral insulin sensitivity. Sitagliptin (a DPP-4 inhibitor) increases incretin levels, enhancing glucose-dependent insulin release and reducing glucagon. Pioglitazone (a thiazolidinedione) improves insulin sensitivity in muscle and adipose tissue.
Tablet for oral use. Take exactly as prescribed, usually with meals to reduce stomach upset. Swallow whole with water; do not crush/chew. Regularly monitor blood glucose/HbA1c as advised.

Prescription-only antidiabetic. Metformin: risk of lactic acidosis—avoid in severe renal impairment; assess kidney function before and during therapy; temporarily stop before/after iodinated contrast or major surgery as advised. Pioglitazone: may cause/worsen heart failure due to fluid retention—contraindicated in NYHA class III/IV; monitor for rapid weight gain, oedema, breathlessness; may increase fracture risk; monitor liver enzymes. Sitagliptin: rare pancreatitis/hypersensitivity—seek care for severe abdominal pain or rash. Use caution in elderly, dehydration, liver disease; maintain diet/exercise.
