Treatment of chronic heart failure (particularly heart failure with reduced ejection fraction, HFrEF) to reduce the risk of cardiovascular death and hospitalization.
Sacubitril inhibits neprilysin, increasing levels of beneficial natriuretic peptides leading to vasodilation and reduced fluid overload. Valsartan blocks angiotensin II (AT1) receptors, reducing vasoconstriction and aldosterone effects. Together they lower cardiac workload and improve heart failure outcomes.
Take as prescribed by the doctor, usually twice daily, with or without food. Swallow the tablet whole with water; do not crush or chew. Dose is titrated based on blood pressure, kidney function and tolerance; do not stop abruptly without medical advice.

Contraindicated with a history of angioedema related to ACE inhibitors/ARBs. Do not use together with an ACE inhibitor; maintain a 36-hour washout when switching from an ACE inhibitor due to angioedema risk. Use with caution in low blood pressure, dehydration, kidney impairment, hyperkalaemia, severe liver disease; monitor BP, serum potassium and renal function. Avoid in pregnancy; can harm the unborn baby. Seek emergency care for facial/lip/tongue swelling or breathing difficulty.
